• Distribution Election Form
  Distribution Election Forms received between 1st and the 15th of each month are issued on the last business day of that month. Distribution Election Forms received between the 16th and last business day of the month are issued on the 15th of the following month.
• Minimum Required Distribution Election Form
  Distributions are issued on the 15th and last business day of each month.
• Enrollment and Beneficiary Designation Form
• AUL Investment Change and Transfer Form
  This form may be used only if your funds are invested in AUL’s variable product and your Plan allows for diversification of funds.
