Participant Forms

• Acknowledgement and Designation of Beneficiary Form with SSA-1945
  This form may be used for name or address changes as well.
• Distribution Request Form
  If your ending balance is $5,000 or more, you are not eligible to withdraw your funds until termination of employment.
  Processing takes approximately 60-90 days from the date that we receive your completed Distribution Request Form. Once processed and approved, checks are issued between the 15th and last business day of the scheduled payout month.
  Please note the following school districts have specified distribution cycles:
  Seguin ISD (quarterly)
  Laporte ISD (annually)
